Biomechanics. The stupidity of “Pain science” in Low Back Injury Rehab. It is hard to comprehend the abdication of responsibility by Physiotherapy educators in Australia from the history of clear science of the mechanisms of Low Back Injury. Beginning in 1680 when Borelli published what is considered to be the first book on Biomechanics we have had layer upon layer of concrete science built upon that. Whether it is neurophysiology, Anatomy, biology, physics they all contribute to our understanding of the mechanisms of Low Back Injury. In 1934 disc injury and low back pain were clearly demonstrated by Mixter and Barr. Panjabi published his work on Neutral Zone theory in 1984. It is not questioned and is the basis of our ACL Rehab, but educators who used it to validate their TA/Multifidus theory are now silent (maybe because TA/Multifidus was not a force couple). So 400 years after Biomechanics was first used to demonstrate movement and that we use it to validate peripheral joint Rehab, Physios are being told that Back Pain is a problem in the way a patient thinks about their pain! WTF. We have regressed 400 years because Australian academics are not capable of biomechanical assessment of Low Back Pain and so it’s easier to say it’s in the patient’s head.
